Spending on income security and the stimulus will soon drop. In the coming decades a much larger cost looms.

"The biggest long-term challenge is how we will pay for health care," says Alan Viard, budget expert at American Enterprise Institute.

As the cost of medical treatment rises, the bill for Medicare, Medicaid, and other services makes Social Security look cheap by comparison.
